Hospice care in Antigua initially started in palliative care being offered in homes when it was discovered that there were no available options for the people diagnosed with terminal diseases. In response to this need, St John launched a palliative home care service in 2005. The St John Hospice is the only one in the Eastern Caribbean region to provide palliative care for the terminally ill in homes.

Currently there are 15 staff employed and many volunteers providing care to patients as well as maintenance to the facilities. Since 2007, they have assisted 200 people.

Due to the Covid pandemic the Antigua volunteer program has been put on hold but they continue to partner with a nursing school on the island giving students the opportunity to do their practical training at the hospice.
