Hospice care in Antigua initially started in palliative care being offered in homes when it was discovered that there were no available options for the people diagnosed with terminal diseases. In response to this need, St John launched a palliative home care service in 2007. The St John Hospice is the only one in the Eastern Caribbean region to provide palliative care for the terminally ill in homes.

Currently there are 14 staff employed and many volunteers providing care to patients as well as maintenance to the facilities. Since 2007, they have assisted 200 people.
